Ashbury Alligators Registration

All forms and payments (except Worker Sign-Up Sheet & Meet Attendance
Sheet) are due by 8:00 pm Friday, April 16 (3532 Lawrence Drive, place in
envelope on door). Registration will be considered late and incur a late fee if
any or all payments or forms are missing past the registration deadline.

Please print the following forms to register for the Ashbury Alligators Swim
Team:

Registration Instructions/Check Sheet

1. Swimmer & Emergency Information Form (one per family)

2. Family Commitment Agreement (one per family)

3. Park District Waiver Form (TWO per SWIMMER)

4. Personal Checks made out to "Ashbury Alligators" (TWO checks per family)

REGISTRATION FEES:
Each Swimmer = $150

LATE FEES (Registering after 4/16/10 deadline):
1 Child late fee = $50
*Each addition child late fee = $20

(See #2 Family Commitment Agreement):
WORKER Deposit:
Each Family = $100 (Note: Check will not be cashed unless there is a
violation of Family Commitment Agreement. If check is not cashed, it
will be destroyed after City Meet.)
